MEMBERSHIP

Membership of the Dialysis & Transplant Association of Victoria Inc. is open to any person who is involved with renal conditions, including family, carers and friends. Membership costs only $10 per family, and is due on the first of July each year.

To apply for a new membership, please print out the New Members Form, (a PDF file)* and send it, with your remittance to The Treasurer, D.A.T.A. Inc., P.O. Box 165, Blackburn 3130.

To renew you membership, just print out the Renewal Form, (a PDF file)* and send it, with your remittance to The Treasurer, D.A.T.A. Inc., P.O. Box 165, Blackburn 3130.

We have credit card facilities: only provide your details on the printed form.
